Our skilled team of healthcare providers offers comprehensive primary care services, including routine check-ups, preventive screenings, urgent care, and management of chronic conditions.

Health Education

 We believe in the power of knowledge. Through health education programs, workshops, and community outreach initiatives, we empower individuals and families with information and resources to make informed decisions about their health and well-being.


Embracing modern technology, we offer virtual consultations and telehealth services, allowing rural residents to connect with healthcare professionals remotely and receive expert medical advice without the need for travel.

Preventive Care

Our focus on preventive healthcare aims to reduce the incidence of diseases through regular screenings, immunizations, health promotion campaigns, and lifestyle counseling.

Specialty Services

We will also provide HRT, diabetic management, aesthetics, and weight loss. We aim to bring these services to rural areas, minimizing the need for long-distance travel.

Community Engagement

We actively engage with the local community, organizing health fairs, support groups, and volunteer opportunities that foster a sense of belonging, build social connections, and promote overall well-being.

With nine years of experience as an LPN, Jaclyn has developed a strong background in primary care, urgent care, women’s health, and in-office procedures. Her enthusiasm for serving her patients is evident in her commitment to her local community and her dedication to providing quality care that results in optimal patient outcomes. Jaclyn’s experience in rural healthcare has also given her a unique perspective on the challenges facing underserved populations. She graduated with honors from Tennessee College of Applied Technology with degrees in nursing and business systems technology, demonstrating both her clinical expertise and her business acumen. Jaclyn resides in her hometown of Greenfield with her three children, Mariah, Jace, and Londyn.

Beth Graves is a nurse practitioner who has been practicing for eight years. She is a board certified family nurse practitioner and has experience treating patients of all ages. She received her Master’s of Science in Nursing (MSN, FNP-C) from Austin Peay State University in 2015. Beth believes in treating the whole patient with a focus on wellness and disease prevention.

Beth specializes in diabetic management, weight loss, and women’s health. She is certified in Nexplanon insertion and removal and performs routine women’s health exams. Her pediatric experience includes developing a thesis with treatment plans for pediatric clients with ADHD and comorbid conditions such as anxiety. Beth also has experience in urgent care treatment, including suturing, EKG and radiology interpretation, and treatment of acute illnesses.

Before becoming a nurse practitioner, Beth worked as a registered nurse for six years, gaining experience in the operating room, home health and hospice care. She graduated from the University of Tennessee at Martin in 2009 with a Bachelor’s of Science in Nursing (BSN, RN).

Beth and her daughter Maggie live in Greenfield, Tennessee, and enjoy spending time with family and friends, attending First Baptist Church, and spending time at the lake and beach together. Beth is passionate about serving her community and her patients.

Audrey has a strong background in healthcare management and is committed to helping her local rural community receive accessible, affordable, and quality care. With seven years of experience in the healthcare industry, she has developed a passion for improving patient outcomes and reducing gaps in care. Audrey’s administrative skills are strong, and she has extensive knowledge and experience with Medicare and Medicare Advantage plan coverage, health insurance contracts, benefits, Medicaid, and commercial health insurance products. She also has experience working in value-based care.

Audrey graduated from Jackson State Community College with an Associate of Applied Science in Radiography. She then went on to receive her Bachelor of Science in Healthcare Administration from Trevecca Nazarene University. Prior to her work in healthcare management, she worked as a radiologic technologist in both the hospital and clinic settings.

Audrey lives in Greenfield, Tennessee, with her husband Jeffery and son Major. In her free time, she enjoys cooking, spending time with family, and traveling. She and her family attend First Baptist Church in Greenfield and enjoy participating in church events. Audrey is committed to growing as a healthcare leader and continuing to make a difference in her local community.
