Magnolia Clinic TN offers a range of medical and aesthetic services, including but not limited to:

  • Primary Care: Routine check-ups, preventive care, chronic disease management, and general wellness.

  • Aesthetic Services: Botox, Dysport, dermal fillers, HydraFacials, brow and lash treatments, and other cosmetic procedures.

  • Medical Weight Loss: Customized programs, including medication management, nutritional guidance, and lifestyle consultations.

2. Eligibility & Patient Responsibilities

  • You must be 18 years or older to receive aesthetic treatments unless accompanied by a legal guardian.

  • Patients must provide accurate and up-to-date personal, medical, and insurance information.

  • You are responsible for following pre- and post-care instructions provided by our team.

3. Appointments & Cancellations

  • Appointments can be scheduled online or by calling our office at 731-281-7501.

  • We require at least 24 hours' notice for cancellations or rescheduling.

  • Late arrivals of 15 minutes or more may require rescheduling.

4. Payments & Insurance

  • Primary care services may be covered by insurance. Patients are responsible for co-pays, deductibles, and non-covered services.

  • Aesthetic and weight loss services are not typically covered by insurance and must be paid out-of-pocket.

  • Payment is due at the time of service. We accept cash, credit/debit cards, and approved financing options.

5. Medical Disclaimer

  • Our providers offer medical advice based on individual consultations. Results for aesthetic and weight loss treatments vary.

  • Treatments such as Botox, Dysport, and weight loss programs have potential risks, which will be discussed during your consultation.

  • Magnolia Clinic TN is not liable for adverse reactions if patients do not follow post-treatment care instructions.

6. Privacy & Confidentiality

  • We follow HIPAA regulations to protect your personal health information.

  • Your medical records will not be shared without your written consent, except as required by law.

7. Refunds & Service Changes

  • Medical and aesthetic services are non-refundable once performed.

  • Unused prepaid services may be eligible for a credit toward other treatments.

  • All treatments and services are subject to availability and may be adjusted based on patient needs.

8. Right to Refuse Service

Magnolia Clinic TN reserves the right to refuse service to any patient if:

  • There is non-compliance with medical recommendations.

  • There is disruptive or inappropriate behavior toward staff or other patients.

  • The requested treatment is deemed unsafe or medically unnecessary.

9. Changes to Terms of Service
